General description
Total serum calcium consists of several fractions. A large fraction is bound to inorganic and organic anions, mainly albumin. Therefore, the total serum calcium measurement may underestimate the level of biologically active calcium, especially in patients with low albumin levels. In these patients the Payne formula is commonly used to adjust the measured total serum calcium concentration.
The formula was published in 1973 but has since then been shown to be inaccurate in certain patients, especially patients with renal failure; in these cases, measurement of ionized calcium level is recommended.

Formula
Corrected Calcium [mmol/L] = (0.02 * (Normal albumin - Serum albumin)) + Serum calcium
Normal albumin = 40 g/L = 4 g/dL
Corrected Calcium [mg/dL] = (0.8 * (Normal albumin - Serum albumin)) + Serum calcium
Normal albumin = 40 g/L = 4 g/dL
