General description
Upper respiratory tract infections are one of the most common reasons for visiting a family physician and receiving an antibiotic. But only 10 % to 20 % of sore throat presentations in general practice actually culture positive for group A Streptococcus (GAS), and are with that an appropriate indication for prescribing antibiotic therapy. Taking that into account, Centor score and other clinical prediction rules have been proposed as a way to increase the accuracy of clinical diagnosis of GAS infection and to decrease the unnecessary antibiotic prescriptions.
Centor (McIsaac modification) score estimates probability of group A streptococcal pharyngitis in children and adults and suggests the management course depending on the calculated score value. The score is based on the combination of 5 variables: age, temperature, presence of cough, tender anterior cervical adenopathy and tonsillar swelling or exudate.
The Centor score and its McIsaac modification have been externally validated and are endorsed by American and European guidelines for the diagnosis and treatment of GAS pharyngitis as one of the suggested clinical prediction rules in adults.
