General description
The Hamilton Depression Rating Scale (abbreviated HDRS, HRSD or HAM-D), was published in 1967 and is one of the most commonly used instruments for assessing depression. It is a clinician-rated measure devised for use on patients already diagnosed with affective disorder of depressive type. HDRS has been used extensively in clinical research and in ordinary clinical practice for assessment of the severity of depression, changes in its severity over time, and efficacy of treatment. It should generally be used with the corresponding structured interview. Its value depends entirely on the skill of the interviewer in eliciting the necessary information.

The original version of HDRS is also one of the most commonly used versions; it contains 17 items pertaining to symptoms of depression experienced over the past week. However, the original study did not include thresholds for depression severity stratification. One commonly used set of thresholds was later developed by Mark Zimmerman et al. It can be used to classify patients into mild (8-16 points), moderate (17-23 points), and severe (≥ 24 points) depression.

The Hamilton Depression Rating Scale has been externally validated and is recommended by the European Psychiatric Association (EPA) and American Psychiatric Association (APA) guidelines as one of the suggested assessment instruments for ongoing clinical monitoring of depression and treatment effect assessment.
