General description
The MIDAS (Migraine Disability Assessment) questionnaire was developed by Stewart and Lipton in 2002 to measure headache-related disability and improve doctor-patient communication about the functional consequences of migraine. The questionnaire is based on five disability questions that focus on lost time in three domains: schoolwork or work for pay; household work or chores; and family, social, and leisure activities. The MIDAS score is derived as the sum of missed days due to a headache over a 3-month period in the three domains. The resulting scores stratify the patients into 4 grades ranging from little or no disability through mild or moderate disability to severe disability.
The MIDAS questionnaire was externally validated and has demonstrated reliability. It is endorsed by The American Headache Society in their Consensus Statement as a validated instrument for measuring response to preventive treatment and it is the most frequently used disability instrument in migraine research.
